Appointments:

NOTE: Failure to provide accurate medical/insurance information is illegal, and will result in a $500 penalty.

What must I bring to my appointment?
• Insurance information, list of medications you are currently taking, valid I.D, and co-pay (if any).
• Any MRI/EMG you may have done.
What if I am a new patient?
New patients must fill in an intake forms. Come at least 15 minutes prior to your appointment to fill out the necessary paperwork.
What if I am unable to fill out the paperwork?
If you are unable to fill out any paperwork, you may bring someone along to help you.
What if I am late or cannot make my appointment?
• If you are 15 minutes late for your appointment you will be considered a no-show.
• If you are running late or cannot make it to your appointment, call and reschedule if you have to.
• If you are a no-show for three visits, you will be dropped and we will make no more attempts to reschedule your appointment.
Why do you need my credit card on file?
We need an alternative form of payment on file, even if you use Medicaid

What if my insurance is expired or I have no insurance?
It is your responsibility to have valid insurance. You will be responsible for paying in full.
Do you accept out of state Medicaid?
No.
What if my Medicaid is not eligible?
You are then financially responsible.
Fees and Payments:
What if I do not know my copay?
• We will charge $100.
• If you copay is lower, we will send you a refund within $30 to the address we have on file.
What forms of payment do we accept?
We accept cash, all major credit cards.
How can I pay my bill?
In person, over the phone, via mail to our address, or online(coming soon).
When are payments due?
• Payments are due within 90 days.
• A service fee of $25 per month is imposed thereafter.
What if I cannot afford full payment?
We consider payment plans, as long as no less that 1/12th of the payment will be made in one year, and payments are auto-debited.
What if I owe a balance?
We will send two bills, email, text and call you three times each to remind you of your obligation to pay. If we do not hear back from you within 90 days and you make no attempts to pay your balance, we reserve the right to take you to collections, including, small claims court.
