What is Cyst Excision and Removal?

Cyst excision and removal is a minor surgical procedure performed to eliminate abnormal fluid-filled sacs, most commonly ganglion cysts, from the hand or wrist. These cysts often arise from joints or tendons and may fluctuate in size or cause discomfort.

Causes

Cysts form due to:

  • Joint or tendon irritation – leading to fluid buildup

  • Repetitive use or trauma – micro-injury to soft tissues

  • Underlying arthritis – especially in the fingers and wrist

  • Herniation of synovial tissue – forming a sac filled with synovial fluid

Ganglion cysts are the most common and can appear on the top or bottom of the wrist, base of the fingers, or at the fingertips.

Signs & Symptoms

Patients with a hand or wrist cyst may experience:

  • Visible lump that may be soft or firm

  • Pain or aching with motion or pressure

  • Limited range of motion in nearby joints

  • Numbness or tingling if nerves are compressed

  • Cosmetic concerns or irritation with activity

Some cysts may be asymptomatic but persistently visible.

Diagnosis

  • Physical examination – assessing size, location, mobility, and tenderness

  • Transillumination test – using a light to confirm the cyst’s fluid-filled nature

  • Ultrasound or MRI – when deeper or atypical cysts are suspected

We also rule out other masses such as tumors, infections, or bony abnormalities.

Treatment Options

Non-Surgical Management

  • Observation – Many cysts resolve on their own

  • Aspiration – Drawing fluid out with a needle, often followed by steroid injection (higher recurrence rate)

Surgical Excision

  • Recommended if the cyst is painful, recurrent, or functionally limiting

  • Performed under local or regional anesthesia

  • Involves:

    • Making a small incision over the cyst

    • Removing the cyst along with its stalk (connection to joint/tendon sheath)

    • Ensuring complete excision to reduce recurrence

  • Outpatient procedure with minimal downtime

Postoperative Care

  • Light dressing or splint may be used for protection

  • Keep area clean and dry for several days

  • Resume gentle movement within a few days

  • Return to full activities in 2–4 weeks, depending on cyst location and healing

Associated Risks

Though rare, risks may include:

  • Recurrence of the cyst

  • Scarring

  • Infection

  • Joint stiffness

  • Injury to nearby nerves or vessels

Prognosis
